It is a word used loosely, so here is what it means in practice: there is no factory, no production line and no stock. One person bakes, in a home kitchen, after your order comes in. Ingredients are bought in small batches rather than in bulk sacks, and nothing is baked speculatively hoping it sells. If you order a chocolate cake on Tuesday for Thursday, the flour and eggs for it are typically bought on Wednesday.
